Student Health Clinic

 

The Student Health Clinic will close for the Thanksgiving break beginning at 5:00 p.m. on Friday, November 17, 2017until Monday November 27th at 8:00 a.m. Medical and Pharmacy services will not be available during the break.  The Student Health Clinic will re-open for Medical and Pharmacy services at 8:00 a.m. on Monday November 27, 2017.

A Student Health Clinic receptionist will be available to assist you Monday-Wednesday, November 20-22nd 8:00 a.m.-5:00 p.m. please call 319-273-2009 and choose option #1.

If you have an urgent medical issue at any time during our closure, you may be seen at one of the local urgent care centers. Please visit the Student Health Clinic hour’s webpage at health.uni.edu /about/hours for information and locations.

If you have immunization records to submit they can be faxed to: 319-273-7030. If you have questions regarding your Student Health Insurance Plan premium, please email connie.potter@uni.edu or call 319-273-7736.

FLU SHOT CLINIC:  The Student Health Clinic will host a walk-in flu shot clinic on Thursday, November 30th from 8:30-4:30.  There is a limited number of flu shots remaining.  Please plan to attend!

 Each Wednesday of the month, from 2:00-3:00pm, visit the Student Health Clinic Conference Room for a Wellness Wednesday program. No registration required. For more information, email joan.thompson@uni.edu

Clinic Forms

Health Insurance Information Form

You may provide health insurance information to the Student Health Clinic prior to an appointment or if your insurance has changed.  Please complete the Insurance Information Form and return along with a copy of your insurance card (front and back). Students: If you are 18 or older, make sure you sign the form.

Mandatory Health Requirements

Immunization to measles, mumps, and rubella (MMR) is a requirement for all students who attend UNI.  To comply with this requirement, all students must provide records documenting proof of TWO measles, mumps, and rubella (MMR) vaccinations. Immunization dates must include month, day and year.  We also must collect information on the status of student's vaccinations against meningococcal disease.  Students can provide both the MMR and Meningitis vaccine on the Health Requirements Form.  A photocopy of your immunization record should be attached to the Health Requirement Immunization Verification form.

Treatment of Minors

For students under 18 years of age, a parental or legal guardian authorization for medical treatment form must be on file in our office in order for you to receive prompt care and treatment should the need arise.  Please complete and return the Consent for Treatment of a Minor (PDF) form

Request Medical Records

To request a copy of your health record, please complete and return the Consent to Release or Receive Medical Information or Consent to Release or Receive Mental Health Information (PDF) form.  All information must be completed in order for your request to be processed.  

* Please allow 48-72 hours from receipt of the signed release form for your health information records to be processed.

Psychiatric Care Financial Assistance Form

Financial assistance may be available for your psychiatric care provided at the Student Health Clinic.  You can complete the financial assistance request form and return to the Student Health Clinic.
