Covid Vaccine Booster Clinics 

The UNI Student Health Clinic will host a Pfizer-BioNTech Covid-19 booster on-campus clinic for eligible students, faculty, and staff on Thursday, December 2nd.  This clinic is by appointment only.  You must call the UNI Student Health Clinic Covid-19 phone line at (319)273-2100 to schedule an appointment.  No walk in appointments are available.

2021 Flu Shot Clinics

UNI Student Health Clinic will host two additional walk-in clinics at the Student Health Center.  Face coverings are required to receive a flu shot.  The Student Health Clinic will file a claim with student’s health insurance companies or for those without health insurance the cost of the flu shot is $30.  Thursday, December 9th 9:00am-11:00am and Friday, December 10th 1:00pm-3:00pm

Covid-19 Vaccine information

The Student Health Clinic will be offering Covid-19 vaccines throughout the fall 2021 semester on Thursday afternoons from 1:00 pm - 3:00 pm and Friday mornings from 9:00 am - 11:00 am.  Please call the Covid-19 phone line at (319) 273-2100 to set up an appointment.  Walk-ins are welcome.  Students must remain at the clinic for 15-30 minutes after receiving the vaccine to monitor for any unexpected changes in their health or allergic reactions. 

All members of the campus community are strongly encouraged to receive a vaccination against COVID-19. Vaccination is the leading prevention strategy to protect individuals from COVID-19 disease and end the COVID-19 pandemic. Prior to your scheduled vaccine appointment  please print and complete the Covid Vaccine Consent Form.  You will need to bring this form with you to your appointment.

Health Insurance Information Form

You may provide health insurance information to the Student Health Clinic prior to an appointment or if your insurance has changed.  Please complete the Insurance Information Form and return along with a copy of your insurance card (front and back). Students: If you are 18 or older, make sure you sign the form.

Mandatory Health Requirements

Immunization to measles, mumps, and rubella (MMR) is a requirement for all students who attend UNI.  To comply with this requirement, all students must provide records documenting proof of TWO measles, mumps, and rubella (MMR) vaccinations. Immunization dates must include month, day and year.  We also must collect information on the status of student's vaccinations against meningococcal disease.  Students can provide both the MMR and Meningitis vaccine on the Health Requirements Form.  A photocopy of your immunization record should be attached to the Health Requirement Immunization Verification form.

Treatment of Minors

For students under 18 years of age, a parental or legal guardian authorization for medical treatment form must be on file in our office in order for you to receive prompt care and treatment should the need arise.  Please complete and return the Consent for Treatment of a Minor (PDF) form

Request Medical Records

To request a copy of your health record, please complete and return the Consent to Release or Receive Medical Information or Consent to Release or Receive Mental Health Information (PDF) form.  All information must be completed in order for your request to be processed.  

* Please allow 48-72 hours from receipt of the signed release form for your health information records to be processed.

The Student Health Clinic is an AAAHC accredited facility. We participate in a voluntary site survey to measure the quality of our services and performance against nationally recognized standards of The Accreditation Association for Ambulatory Health Care.
